It is a beautiful tradition to remember those family and friends who are no longer with us. We would be honored to continue listing your loved ones in the United Jewish Center Book of Memory for our 5786 Yizkor services.

Please review your last year’s listing(s) and add anyone new you would like to honor. Kindly fill out this form with payment for the appropriate amount by August 22, 2025, to confirm acceptance.

Remember that we continue to offer the opportunity to list names in perpetuity for a one-time fee of $360.00 per name or $36.00 per name annually for the 2025-2026 Memorial Book.
